All our senior housing programs follow a Wellness Model with the following components:
- Comprehensive care coordination strategy: initial and ongoing assessment, the development of a service plan, and routine, as well as episodic review of the plan;
- Participation of the resident, family, and staff in the development and revision of the service plan;
- Delivery of a high-touch, high-tech service plan by well-trained, caring, experienced staff;
- Day-to-day management, responsibility, and accountability for the operation of the program;
- Implementing a communication strategy among the community and LifeStyle Options staff that ensures thorough knowledge of resident status;
- Facilitating and assisting in the transfer to a higher level of service when the need arises;
- Assuring aggressive oversight of the residents' medication regimens;
- Encouraging and facilitating residents' use of the services that may benefit them;
- Providing 24/7/365 staffing as needed;
- Coordinating and ensuring the use of skilled nursing, therapies, and other strategies to achieve the highest level of functioning possible;
- Assuring that the episodic/temporary health, I/ADL, nursing needs of the residents are met;
- Allowing for "managed risk", a process of negotiating an acceptable risk level between the resident, family members, the community and LifeStyle Options, Inc; and
- Responsibility for all business functions including payroll, billing, insurance, taxes, accounts payable, accounts receivable, and management financial reporting.
