PROFESSIONAL MEDICATION MANAGEMENT
Almost 40 percent of all adverse drug reactions reported each year involve people over 60 years old. Older adults take more medication now than ever before. The average person over 65 takes between 2 and 7 prescription medications daily.
With aging, many body processes are altered and often slow down, affecting the way medications are absorbed, distributed, metabolized and excreted. These elements combine to create a greater risk of drug interactions, drug/food interactions, and side effects. Some medications will not work or have dangerous side effects when taken with other medications. Cognitive difficulties also interfere with the ability to remember when, how and if to take medications.
LifeStyle Options, Inc. offer Professional Medication Management programs to meet these needs.
Option I. Professional Medication Management:
In this option, we take complete responsibility for oversight of an individual’s medications. Designed for individuals with multiple medications and/or cognitive or physical challenges that prevent good medication compliance., it works like this:
- First, a Registered Professional Nurse (RN) notifies the individual’s physician(s) of the service and verifies the medication regimen.
- A licensed pharmacist reviews the medication regimen for interactions and side effects.
- Medications are set up weekly in appropriate containers.
- The RN handles all refills, changes and medication updates.
Option II. Medication Set-Up and Reminders
Option II focuses on accurate dosage and timely compliance. Designed for those individuals who would benefit from an organized system and regular reminders, Medication Set-Up and Reminder service can be purchased as a package or can be purchased separately as either Medication Set-Up only or Medication Reminders only. Medication Set-Up and Reminders service includes:
- Close communication with the family regarding any issues
- Medications are set up weekly in appropriate containers by a nurse
- Reminders by qualified staff on a regularly scheduled basis

PROFESSIONAL GERIATRIC CARE MANAGEMENT SERVICES
Overview: Increasing numbers of older people are living to advanced ages in the United States. Most older people prefer to remain in their homes as they age, but finding the right long-term care providers and services can be difficult. Consumers often do not know exactly what kind of help they need, how to find and arrange for services, and how much they must pay for services. A knowledgeable Professional Geriatric Care Manager (PGCM) will, for a fee, assess a client's needs, arrange services, and monitor care on an ongoing basis.
PCGM services can be a valuable option for older persons and their families who can afford such services. From assessment of needs to advice on available resources, a well-trained care manager can help individuals to remain as independent as possible, to live in one's home with assistance, or to assess other living arrangements such as supportive housing or assisted living facilities.
What is a Professional Geriatric Care Manager?
A Professional Geriatric Care Managers (PGCM) is a health and human services specialist who helps families who are caring for older relatives*. The PGCM is trained and experienced in any of several fields related to long-term care, including, but not limited to nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care.
The PGCM assists older adults in attaining their maximum functional potential. In addition, the PGCM is an experienced guide and resource for families of older adults and others with chronic needs, including helping those suffering from Alzheimer’s Disease or exhibiting symptoms of dementia.
* Consumers must have confidence that people who say they can perform this work have the right education and training to do so. Many GCMs are licensed nurses or social workers, but these licenses do not necessarily guarantee an ability to address a client's financial or legal questions or provide comprehensive knowledge of all service options. In addition, many geriatric care managers are not members of any licensed profession. In most states, any person can call himself or herself a geriatric care manager and offer services to the public. Consumers can face the possibility of uninformed advice or outright fraud. Web sites such as the National Professional Geriatric Care Manager’s Association can be very helpful in assessing professional competence.
How Do You Know if You Really Need a Professional Geriatric Care Manager?
When considering to engage the services of a PGCM, you should evaluate whether you have the time, inclination, or skills to manage the challenges of geriatric care. If you are not sure, ask a trusted advisor to help you decide if an elder care expert may be helpful. Enlisting the support of other family members to consult a professional is a good way to build a consensus on the solutions.
What Can a Professional Geriatric Care Manager Do for You?
- Conduct care-planning assessments to identify problems and to provide solutions.
- Screen, arrange, and monitor in-home help or other services, including assistance in hiring a qualified caregiver for home care.
- Provide short- or long-term eldercare assistance for those engaged in local or long distance caregiving.
- Review financial, legal, or medical issues and offer referrals to geriatric specialists. Act as a liaison to families at a distance, overseeing care, and quickly alerting families to problems – especially important when families are engaged in long distance caregiving for a loved one.
- Assist with moving an older person to or from a retirement complex, assisted care home, or nursing home.
- Provide consumer education and advocacy.
